1 Must be scientifically based or modelled as far as reasonably practical; but values adopted eg stress levels, must be based on risk, probability, practicability, and experience.

2 This system is aimed to:

  a) encourage and facilitate development of new K from ideas, experience and R&D; build on existing K ie keep it up-to-date; and identify missing K;   and

  b) make K input and output available to all free at any time anywhere

3 Must recognise that K grows and changes with time and circumstances. Also that it is never complete but usually sufficient and accurate enough for most purposes

4 "A little K is dangerous". Yes! But no knowledge can be worse.

5 Documented K should have suitable reference for further info and to acknowledge the originator.

6 Experience can be a great provider and teacher of K if we use it properly!
Hence to maximise K development, the following should be done as far as practical -
seek, identify and report NEW valuable information and K gained in the course of experience in:

 a) inspections and investigations (private, company, industry or government);
 b) all failures, accident or incident investigations;
 c) R&D;
 d) Coroners' enquiries and Royal Commissions; and
 e) Industrial and other court actions
 f) successful trials, designs, manufacture etc


Having this as a clear subsidiary objective, make maximum gain for the nation with little or no extra effort - probably less than 0.1% all others costs. Only clear factual information need be the minimum.

Also, disclosure of this quickly without the overall claim of confidentiality and risk of prosecution in its release and use, will facilitate progress. This applies specially to incidents where workers and the public has been at risk. eg. what was the failure mode and technical causes of th failures
